Vitamins are substances that play an essential part in animal metabolic processes, but which the animals cannot synthesise. In their absence the animal develops certain deficiency diseases or other abnormal conditions. Vitamins are chemicals other than proteins, carbohydrates, fats and mineral salts that are essential constituents of the food of animals. Although certain animals can synthesise certain vitamins and all animals needing vitamin D can manufacture it from ergosterol in the presence of u.v. Small amounts of vitamins are essential for the regulation of all bodily processes. With the exception of vitamin D, the human body cannot make its own vitamins, and some cannot be stored. Vitamins must therefore be obtained from a food on a daily basis. A person’s diet must provide all the necessary vitamins. 1

Vitamin B12 works with folic acid in many body processes including synthesis of DNA, red blood cells and the insulation sheath (the myelin sheath) that surrounds nerve cells and facilitates the conduction of signals in the nervous system. Severe depletion manifests as pernicious anemia, which was invariably fatal until the discovery of B12 in liver. But long before anemia sets in, other conditions may manifest, most often neurological problems (numbness, pins and needles sensations, a burning feeling in the feet, shaking, muscle fatigue, sleep disorders, memory loss, irrational anger, impaired mental function and Alzheimer’s) or psychological conditions (dementia, depression, psychosis and obsessive-compulsive behavior). 2

Vitamin B12 is an essential water-soluble vitamin that is commonly found in a variety of foods such as fish, shellfish, meat, and dairy products. Vitamin B12 is frequently used in combination with other B vitamins in a vitamin B complex formulation. It helps maintain healthy nerve cells and red blood cells and is also needed to make DNA, the genetic material in all cells. Vitamin B12 is bound to the protein in food. Hydrochloric acid in the stomach releases B12 from protein during digestion. Once released, B12 combines with a substance called intrinsic factor (IF) before it is absorbed into the bloodstream. 3

The human body stores several years’ worth of vitamin B12, so nutritional deficiency of this vitamin is extremely rare. Elderly are the most at risk. However, deficiency can result from being unable to use vitamin B12. Inability to absorb vitamin B12 from the intestinal tract can be caused by a disease known as pernicious anemia. Additionally, strict vegetarians or vegans who are not taking in proper amounts of B12 are also prone to a deficiency state. 4

Vitamin B12 is needed for cell division and blood formation. Neither plants nor animals make vitamin B12. Bacteria are responsible for producing vitamin B12. Animals get their vitamin B12 from eating foods contaminated with vitamin B12 and then the animal becomes a source of vitamin B12. Plant foods do not contain vitamin B12 except when they are contaminated by microorganisms or have vitamin B12 added to them. Thus, vegans need to look to fortified foods or supplements to get vitamin B12 in their diet. Although recommendations for vitamin B12 are very small, a vitamin B12 deficiency is a very serious problem leading ultimately to anemia and irreversible nerve damage. Prudent vegans will include sources of vitamin B12 in their diets. Vitamin B12 is especially important in pregnancy and lactation and for infants and children. 5

Vitamin B6, or pyridoxine, helps the body to absorb and metabolize amino acids, to use fats, and to form red blood cells. Deficiency in the vitamin may result in smooth tongue, skin disorders, dizziness, nausea, anemia, convulsions, and kidney stones. Whole grains, bread, liver, green beans, spinach, avocadoes, and bananas are rich food sources that are high in this vitamin. The RDA ranges from 1.3 to 2 mg depending on age and gender. 6

Because the absorption process is so complicated, and therefore subject to various blocks, many people–particularly the elderly–may develop deficiencies even though they are taking in plentiful B12 in their food. Fortunately, the body absorbs about 1-5 percent of free B12 by a process of passive diffusion. Thus supplementation with large doses of crystalline B12 or with foods extremely rich in B12 can successfully treat deficiencies caused by compromised protein digestion or lack of R-protein, intrinsic factor or pancreatic enzymes. Supplementation with the coenzyme forms methylcobalamin and adenosylcobalamin (the forms found in the cells) can overcome B12 deficiency in the cells caused by lack of, or malfunction of, conversion enzymes. 7

B9, or folic acid, sometimes goes by the name of vitamin M or vitamin B-c. Folic acid enables the body to form hemoglobin. It helps treat anemia and sprue. Good food sources include leafy green vegetables, nuts, whole grains, legumes, and organ meets. However, bear in mind that folic acid is lost when foods are stored at room temperature or cooked. Deficiency is rare, although folic acid is particularly important in pregnancy. Consuming adequate folic acid before and during pregnancy helps prevent neural tube defects in newborns, including spina bifida. The RDA for both men and women is 400 micrograms, but women who are pregnant or planning to become pregnant should consume 600 micrograms a day. When breastfeeding, the recommendation is 500 micrograms. 8

Vitamin B12 deficiency is a cause of megaloblastic anemia. In this type of anemia, red blood cells are larger than normal and the ratio of nucleus size to cell cytoplasm is increased. There are other potential causes of megaloblastic anemia, including folate deficiency or various inborn metabolic disorders. If the cause is B12 deficiency, then treatment with B12 is the standard approach. Patients with anemia should be evaluated by a physician in order to diagnose and address the underlying cause. 9

The patient was started on 1000 micrograms/day of sublingual vitamin B12 supplementation and by November 1994 her level was back up to 517 pg/mL and her homocysteine level was down to 20.3 micromol/L. Further improvements were observed in March 1998 when her vitamin B12 level was up to 629 pg/mL and homocysteine was down to 13.9 micromol/L. The researchers point out that other studies have shown that omeprazole also lowers vitamin B12 levels and conclude that older people on long-term acid-suppressing drugs should be monitored for vitamin B12 deficiency and supplement if necessary. 10

Vitamin B12 (cobalamin) plays a role in the body’s growth and development. It also has a part of producing blood cells, the functions of the nervous system, and how the body uses folic acid and carbohydrates. 11

Many of the following substances have been referred to as vitamins because they were believed to be vitamins at one time, and they are relevant to vitamin nomenclature in that the numbers that were assigned to them form “gaps” in the series of B-vitamin names. Some of them, though not essential to humans, are essential in the diets of other organisms; others have no known nutritional value. Since one of the steps in carbohydrate and fat processing requires B12 for its completion, insufficiency of the vitamin can also affect the movement of carbohydrates and fats through the body. 14

Some patients diagnosed with Alzheimer’s disease have been found to have abnormally low vitamin B12 levels in their blood. However, vitamin B12 deficiency itself often causes disorientation and confusion and thus mimics some of the prominent symptoms of Alzheimer’s disease. Well-designed clinical trials are needed before a strong recommendation can be made. 15
